SITE HOST JOB DESCRIPTION
Objectives:
1.
To help volunteers feel welcome and appreciated
2.
To promote safe practices on site
3.
To have accurate, relevant information on each volunteer in case of
an emergency
4.
To facilitate good communication on site by using name tags
5.
To make sure all volunteers sign in each time they work on the site
Equipment
Needed:
(These items should be on-site in the plastic bin for the construction day)
1.
Duct tape (torn strips will be used for names--suggest red marker for
site leaders, black
marker for volunteers)
2.
Sharpies (red and black) and pens
3.
Clipboards
4.
Volunteer Handbooks
5.
10 Site Sign-In Sheets
6.
50 Individual Volunteer Information Forms
7.
50 Adult Waiver Forms
8.
50 Minor Waiver/Authorization for Medical Treatment Forms
9.
50 Emergency Medical Information Forms
10.
10 Accident Response Procedure and Reporting Forms
11.
1 Construction Safety Guide (to display on work site)
12.
1 A-Z Binder for filing Waiver and Medical Forms (to keep on site)
plus extra volunteer forms

(The following should
be available for Site Host)

Site Host Tasks:
1.
Be on-site from 7:45 am and plan to stay until after lunch is served
2.
Purchase and bring to the site a large bag of ice for the cooler and
add water so that the
volunteers will have plenty to drink throughout the
day. (The CHFH office will provide a
Kroger card or arrangements can be made for
reimbursement.)
3.
Make sure that when people come on site they register and have name
tag
4.
Welcome volunteers, greet them with a smile, thank them for coming
5.
Have each volunteer fill out the below-listed forms (if we do not
already have their
completed forms on file)
a)
Read and sign the Waiver of Liability
b)
Fill out the Emergency Medical Information Form
c)
Fill out the Individual Volunteer Sign-In Sheet (after reviewing
Construction Safety
Guide)
6.
Have each volunteer make a name tag and put it on
7.
Direct the volunteer to the Site Leader, or to the location where
training/orientation will
occur
8.
File Waiver and Medical Forms by first letter of the volunteer's last
name in the A-Z
folder
9.
If you see a volunteer on site without a name tag, ask them to
register with you
10.
Please maintain accurate records – it is CRITICAL that we capture all
volunteer
information along with the site sign-in sheet(s);
this will allow us to retain accurate records
11.
Manage Accident Response Procedure and Reporting Form
12.
Each week please mail Individual Volunteer Information Forms along
with Site Sign-in
